Supporting long-term care
At YMHC, we recognize that the transition from hospital to ongoing care can be a critical juncture in an individual's path to recovery. That's why we've developed our Discharge to Assess (D2A) pathway, a unique and compassionate approach tailored to support individuals who may not be ready to return to their previous place of residence following hospitalization.

Training Education and Employment
One of the cornerstones of our services is to actively motivate and assist our service users in enrolling in training, education, or employment opportunities. This not only helps them establish a routine but also equips them with valuable skills such as functional and vocational skills, as well as interpersonal communication.
